2. The item number(s) you wish to return
3. The condition of each item (unopened, opened, used, damaged, other - please describe)
4. The condition of the item packaging (unopened, damaged, incomplete, other - please describe)
5. The reason you are wanting to return it
6. If you beleive it is defective, have you contacted the manufacturer for an exchange?
7. What would you like when you return it (exchange for a different item or store credit)
